Key elements to a fulfilling healthy life
You will need to print out this evaluation to fill it out.
This evaluation is an opportunity to help you clearly define your current concerns and goals, and even more importantly, to determine your next areas of focus while acknowledging what's going well in your life.
There are five main areas in life that need to be in balance for you to feel your best: health, career, relationships/family, personal growth/stress management, and spirituality/service... in other words, body, mind, emotions and spirit.
For the following statements, rate yourself between 1 - 6 for how true the statements are for you, with 1 being never and 6 being always. Notice how you feel when you read the questions and note any thoughts and responses underneath it. Circle the ones that bring up especially strong thoughts or feelings.
You can see if you're balanced within each category and then see how these areas work with each other in your life. (There isn't a "magic" score or number that will validate the quality and balance of your life -- you'll be able to tell how well you're doing as you fill out the questionnaire and see that some sections have a lot of good things going on and others are fairly lowkey. However, for those of you who must have numbers, it's safe to say that the lower your score, the less awareness you have about your health, and the higher the score, the better for your health and well being.
This quiz is an example of how I might work with you in session to assess how balanced your life is in general, and then we'd hone in on the areas that need more support and attention.
Health - Creating optimal wellness ___You are aware of how various foods affect you (i.e., dietary intolerances, food allergies, high or low blood sugar levels) and you avoid foods that bother you.
___You exercise regularly in a way that supports your body type, including at least 3 hours a week of aerobic activity, some strength building and some stretching.
___You eat breakfast, lunch and dinner at regular times.
___If you take nutritional supplements, you select top quality supplements that are in line with your health goals (i.e., a good calcium supplement for bone density, antioxidants to help with stress, etc.) OR ___You eat 3 very healthy meals each day, plus healthy snacks, getting 4-5 servings of organic fruits and vegetables and other foods rich in antioxidants each day and all required nutrients from your food, have no current health concerns, and no family genetic patterns for chronic degenerative disease (such as cancer, Alzheimers, arthritis, heart disease). If you do take top quality nutritional supplements and eat this way, give yourself points for both statements!
___You limit caffeine intake, or avoid it completely.
___You limit or avoid sugary foods, getting required carbs from fruits, vegetables and complex carb sources.
___You do not smoke.
___You do not use recreational drugs.
___If you take prescription drugs, you work closely with your doctor to make sure you're receiving the appropriate dosage and to avoid interactions with other drugs, herbs and foods.
___You avoid soda pop and junk food.
___You limit your alcohol consumption.
___You do not microwave your food.
___You avoid drinking cold/iced water, especially with meals.
___You stay hydrated, drinking enough water to be urinating every 2 hours or so.
___You eat foods with fiber and have regular, healthy elimination (2-3 bowel movements per day).
___You get 7-8 hours of restful sleep each night.
___You spend between 10-20 minutes a day outside receiving sunlight safely, either in the morning or late afternoon to generate vitamin D and revitalize.
___You breathe deeply and consciously at some point(s) during the day.

Career / Money- Quality of life for you and others ___Your job gives you personal satisfaction because what you do makes a difference to at least one other person and you feel appreciated for your efforts.
___ You receive current fair market compensation for your work and your income is sufficient to meet your current expenses and also to save/invest for retirement.
___You have no credit card debt.
___You have 6 months' income saved that is easily accessible to you.
___You view money as a useful tool for achieving your goals, not an end in itself. Your focus and energy is on your work/job/activity, which generates money as a natural consequence.
___You have a healthy relationship with money and pay your bills on time.
___You have more than one source of income, not putting "all your eggs in one basket" so if one were to stop, you could still meet your expenses.
___You feel at home in your work; the environment is supportive and creative.
___Doing your work does not harm you physically or emotionally.
___Others benefit from what you do and you have a direct sense of this.
___Your work provides just enough challenge to be stimulating and encourages personal and professional growth.
___You look forward to your work each day.
___When you finish your day, you're pleasantly tired but not exhausted. You are easily able to recover each night from any stress on the job. You feel recharged and refreshed at the beginning of your work week.
___Your work is in alignment with your core values and beliefs.
___Your work is appropriate for your constitution type. (bonus question)

Relationships - Family, Friends, and the people you meet ___Your "significant other" (SO) is supportive and honest with you. You feel he or she truly cares about your wellbeing and you work together well as partners in life. OR ___You aren't in an "SO" relationship right now and are enjoying the simplicity of setting goals and making decisions for your own life. You are easily able to take care of your own needs and enjoy time alone.
___You have sufficient time to spend in social gatherings that refresh, stimulate and energize you, and do so at least once a week on average.
___The people you associate with in all your environments are positive, upbeat and energetic most of the time.
___You receive some form of healthy, nurturing touch on a regular basis.
___You have close friends you can confide in, bounce ideas off of and feel close to, outside your SO relationship. You can count on these friends in times of crisis.
___You feel that your SO and/or your close friends respect you.
___You have time and energy to spend with your family each day in ways that encourage exchange of ideas, and enhance your relationship with them.
___You are genuinely interested in other people and respect their right to disagree with your opinions.
___You take the time to communicate clearly with others.
___You are happy with the amount of time you spend with family and friends.
___You treat others as you'd like to be treated.

Personal Growth/Stress Management - Time invested in yourself ___You know how to take care of yourself after a stressful day or event to help yourself feel calm, centered, and focused, and you actually take the time to do it.
___You do some form of aerobic exercise -- not just to maintain a healthy cardiovascular system and boost metabolism, but to burn off the biochemicals generated by stress.
___You enjoy "thinking outside the box" to creatively solve problems.
___You follow your intution and your heart in addition to reason to make decisions. You don't base your decisions on what other people think you "should" do.
___You are willing to push out of your comfort zone to achieve your goals.
___You have identified what you're passionate about and incorporate that into your life.
___You live in the present moment, learning from but not dwelling on the past, and work towards achieving your goals in each moment, day by day.
___If you find yourself worrying about the future, you consciously direct that energy into constructive problem solving and action.
___You enjoy learning and applying the information to create a better life for yourself and others.
___You have consciously defined your true values and have achieved clarity by recognizing and eliminating conflicting inner "rules."

Spirituality - Being connected to something greater than yourself ___When you feel overwhelmed, you know how to tap into a greater source of energy outside of yourself and do so. This source is a comfort to you.

___You spend some quiet time each day, meditate, or pray each day to enhance and strengthen your alignment with your chosen source of spiritual inspiration and to center yourself.

___You spend some time outside every day.

Great! You've finished the evaluation. Now, go back and, if you haven't already done so, add up the totals for each section. See how your totals compare with the maximum number of points possible for each section. Then compare your grand total to the maximum points possible (438). The points are meant to satisfy your logical/rational side.
Now, let's look at your feelings and intuition. Go back and find 3 things you'd most like to change and circle them, based on your gut reaction to reading and answering the question.
Then, go back and place a star next to three areas that you were the most pleased about -- questions most likely that rated a 6.
A final note -- this evaluation isn't supposed to be yet another weapon with which to mentally clobber yourself. It can show you in a definitive way what your strengths are and where you might want to make improvements to have a happier, more satisfying life.
